Client Forms
Please download, print, read, and sign all forms below for your first appointment. If you do not have a printer, please notify Dr. Romney Ryan and she will provide you a copy at her office to fill out prior to the first appointment. Please allow 30 minutes to fill out the forms at her office.
The fee is $170 per 50 minute individual or family session.
Longer family appointments for 1 hour, 20 minutes are available for the fee of $253.
Note: The Credit Card Authorization Form is only needed if you want Dr. Romney Ryan to have a credit card on file to bill you for services.
